Application
Application packets may be picked up in the office. In order for registration to be complete the following is needed.
*All students must have an application (new and returning), consent to treatment, student pick-up authorization (one per family), and financial agreement (one per family). For Kindergarten and students entering 7th grade the following will also be needed.
Kindergarten
Proof of immunization (any new students going into any grade will also need to show proof of immunization)
Student medical record (including dental check)
Birth Certificate
7th grade
Student medical record (must include scoliosis exam)
